Several people may encounter such problems where they might get injured or sick to an extent that they can’t go to work for a long period of time. In this case you are expected to inform your organization either through an e-mail or a letter. Write to your employer what problems you have encountered and how you will not be able to visit the workplace for a particular time period. Make sure that your tone is formal. Moreover, attach your medical certificate along with the letter so they can keep a check on the problem you have.

Dear Sir,
I am writing this letter to inform you that while I was on my way back home, I slipped and fell off the stairs. I was carried to the hospital right away and the doctor says that I have torn my knee ligament. This means that I will not be able to come to work from 6th December 2013 to 25th December 2013, after which the Christmas holidays are going to start and I will be returning to work on 1st of January.
Since I have a lot of leaves left in my leave balance, I would like to use those in order to fully recover from my injury. I hope you understand my problem and let me stay home for the aforementioned dates.
For 1 week I will be in bed, but after that I will be able to sit on my computer and you can inform me whether you want me to work from home.
My medical certificate has been attached with this letter. Kindly grant me leave for these dates so that I can fully recover from my injury.
