Sample of Insurance Letter

An insurance letter is written to the insurance company whenever you are trying to claim your insurance over anything. You simply write to them and explain why exactly you are claiming insurance.

Start off by mentioning about yourself; if there is a certain ID that you have, do mention it so they can easily locate your data. Then write about the problem you are encountering. You can write in as much detail as you want since you are trying to fully explain your problem. Make sure that you maintain a professional and formal tone throughout the letter. Moreover, try to sum up everything in the letter so they don’t have to contact you later regarding certain issues. Make sure you convey full information to them in a single letter.

Dear Mr Clyde,

I am writing to you to inform you that I am currently considering fertility treatment and would like to verify about all the benefits which are currently available to me. I am being told that the services which I will avail are going to include a diagnostic work-up with a high possibility of a full treatment. The treatment might not involve an operation but it may include options such as prescription of oral medication such as Comihene citrate. There might also be an option of an additional inject-able medication such as Gonadotropins.  Moreover, an additional intrauterine insemination might also be there such as ICD9 code V26.2; which is mostly recommended.

Please write back to me in order to let me know in writing about the benefits and the limitations that will be present for the following services:

Intrauterine Insemination
Prescription Medication
Laboratory Tests
Ultrasounds

Besides this, it is also requested that you identify and include information in my contract that outlines all these items and the limitations or restrictions that you are willing to put upon them.
